The Sheboygan County End of Life Coalition is a collaborative effort of area organizations and their resources to develop and implement strategies to foster Advance Health Care Planning and other support services in Sheboygan County.
History
The Sheboygan County End of Life Coalition was formed in year 2000 as a collaborative effort to educate the community regarding the physical, emotional and psycho-social care at the end-of- life to promote a sense of dignity and meaning for individuals and their families.
Coalition Activities
The Coalition provides educational opportunities for the community regarding Advance Directives, grief resources and end-of-life options through group presentations or one-on-one meetings. Each presentation is tailored to suit the requirements of the target audience.
