Sharon S. Richardson Community Hospice providing end of life care regardless of one's ability to pay.Mission Statement: Our commitment is to improve the quality of life for patients and their caregivers by supporting their end-of-life choices with dignity and compassion. The Sharon S. Richardson Hospice specializes in providing hospice and palliative care services including grief support and advanced care planning in your home, our Center, or any other location.
 

Key Facts

  • Not-for-profit 501C-3 organization (Federal Tax Identification Number: 04-3794775)
  • Locally operated and Independent from any system, open to physicians from any group, hospital, or clinic – physicians are able to follow their  patients into our programs.
  • End-of-life specialty care is provided in any setting the patient calls home, including nursing homes, assisted living facilities, hospitals and their homes.
  • Hospice Care Team members serve patients and their caregivers/families.
  • Services in the community are provided in a five county, 60 mile radius including: Sheboygan, Manitowoc, Calumet, Ozaukee and Washington counties.
  • Patients are accepted in the Center from all geographic regions of Wisconsin, of all ages and with all types of life-limiting illnesses.

 

Impact

  • Over 1,000 patients and their families have been served since the launch of community services in February 2007. With families large and small in number being included in the service circle – thousands across the region have been touched by care team members.
  • After a successful building campaign of $7 million, community members have responded annually with over $400,000 to support the Patient Care Fund. The Patient Care Fund assists patients who are uninsured, underinsured or whose services are not covered by insurance or Medicare. At this time, 25% of our hospice patients need assistance from the Patient Care Fund to cover a portion of their medical care, room and board at the Center, or both.
