Social Worker MSW- Part-Time

Participation in on-call rotation is required.

In this role the MSW will provide services to patients that address the social and/or emotional factors impacting on their ability to achieve the goals identified in their Plan of Care. Services are provided under the direction of a physician along with interdisciplinary team consultation. In addition, this position is involved with promoting the mission of SSRCH through collaboration with community agencies and organizations and actively participating in special events and community activities


• Masters degree in social work

• Minimum of one (1) year social work experience in a health care setting or equivalent experienc in home care preferred.

• Demonstrates excellent written and verbal communication skills and strong interpersonal skills.

• Demonstrates knowledge of resources available in community/service area.

                        Licensed driver with automobile insured in accordance with state/organizational requirements. Good driving record and reliable transportation.
