Spiritual Counselor


 

Essential Duties:

1. Assessment and coordination of spiritual counseling services to support patients, families, and caregivers.

2. Provision of spiritual counseling to meet the needs in accordance with the patients’ and family’s acceptance of this service and in a manner consistent with the patient and family’s beliefs and desires.

3. Coordination of referrals of family members to non-hospice community programs where appropriate.

4. Facilitation of visits by local clergy, pastoral counselors, or other individuals who can support the patient’s spiritual needs to the highest quality.

5. Development and delivery of programs to the community on hospice, loss, and related issues.

6. 100% Accuracy and timely reporting to meet all federal, state, and SSRCH standards for spiritual care counseling

7. Participates in Interdisciplinary Team Meetings as required.

8. Development and delivery of programs to the community on hospice, loss, and related issues.

9. Participates in on-call counseling support as required.

10. Assist with funeral services when support is requested.

11. Participates in on-call counseling support as required.

12. Assists Psych/Social Team in community outreach efforts.

13. Performs other related duties and responsibilities as deemed necessary.

1. Supervision, support, and evaluation of individuals performing spiritual support services including volunteer support.

Working Conditions

1. Work Requirements: Ability to work reasonable amounts of time beyond the standard work week along with flexible hours necessary to meet patient, family, and organization’s needs. Travel on a daily basis.

2. Physical Requirements: Ability to sit or stand for extended periods of time. Climb Stairs as needed. Manual dexterity and visual acuity to operate computer and other office equipment.
a. Home Care - Locates and drives to and from patients’ homes and to meetings at

other locations as necessary.

3. Mental Requirements: Read, speak, write, and understand English. Must be able to function independently without supervision.

4. Working/Environmental Conditions: Fast-paced changing environment, with multiple directives from several disciplines.

a. Home Care - Requires work in patient homes with no control over environmental conditions.

5. Machines, Equipment, Tools Used: automobile, telephone, fax machine, computer, and printer.

6. Exposures: Potential exposure to communicable diseases, infectious materials, bodily odors, bodily fluids, and other pollutants

7. Personal Protective Equipment Required: Assorted gloves, masks, impervious gowns or aprons.

Professional Qualifications

1. Education Requirement: Pursuing a Bachelor’s Degree from an accredited college or university in the field of religion, theology, psychology, or philosophy- credits qualifying junior status.

2. Certificate/Degree/License Requirements: Valid Wisconsin driver’s license upon hire; renewal as required. Must carry car insurance on vehicle used to perform job and maintain an acceptable driving record. Certified in CPR BLS according to the American Heart Association and/or the Red Cross.

3. Experience Requirements: Must possess the capacity by training and experience in grief or loss counseling to provide for spiritual support of families, including the ability to organize a program of directed care services provided to family members. Will need to possess the exceptional talent required to create and deliver distinguished programs to meet the needs of people who experience loss of any kind

4. Communication Skills: Ability to communicate effectively with people of all socioeconomic backgrounds. Able to recognize and respond to non-verbal cues and communication.
