Hospice is a service that provides End-of-Life Care when a patient has a terminal illness and the possibility of six months or less to live and is no longer seeking aggressive treatment. Dedicated to quality of life, the Hospice Team offers a support system of professional staff and volunteers. These team members assist the patient and their caregivers in meeting physical, emotional and spiritual needs. Hospice affirms life and regards dying as a normal process, while working with patients and families to maintain comfort, relieve distress and gain a sense of peace – making every moment count.

Services are provided to patients of all ages, regardless of their type of illness, physician affiliation, or ability to pay.
As an independent not-for-profit organization, we serve patients from all referral sources. Often, patients and their families are the first to contact us regarding services. Assisted living facilities, hospitals, nursing homes, and home health organizations utilize our specialty services.

All of our operations are headquartered in the Hospice Center building – including all administrative functions, and our Home Hospice Program. This team provides End-of-Life expertise in patients’ homes, nursing homes, assisted living facilities, and hospitals throughout Sheboygan County and into Ozaukee, Manitowoc, Calumet, and Washington Counties. When staying in their home is no longer an option, our Home Hospice patients can make a smooth transition into the Hospice Center. They will have priority for a bed, as needed.

Hospice Services

Understanding Hospice Care - an Introduction

Available for a patient with a terminal illness who has the possibility of six months or less to live and is no longer seeking aggressive treatment. This service is covered by Medicare and Medicaid benefits, insurance and private pay. The Medicare/Medicaid Hospice benefit includes 100% coverage of all visits from members of the Hospice Team, and medication and equipment related to the terminal illness. The levels of care include:
 

  • General (Acute) Inpatient Care – for those patients needing acute pain and symptom management provided in the Hospice Center or a hospital setting.
  • Routine Care – provided in a patient’s home, at a nursing home, assisted living facility, or at the Hospice Center. For Routine Care at the Hospice Center, room and board is paid privately and billed separately to the patient at rates comparable to area skilled nursing facilities. A financial assessment is available, as needed.
  • Respite Care – patients can be admitted to a contracted nursing home or the Hospice Center for up to five days to provide caregivers a short break from their responsibilities.
  • Continuous Care – for patients with a range of acute symptoms needing continuous oversight by a Hospice Registered Nurse.

 

Palliative Care Services

What is Hospice Care?

Available for a patient with a terminal or life-limiting illness who has a prognosis of longer than six months to live and may still be seeking aggressive treatment. Their services are covered by private insurance and private pay. Palliative Care includes access to the entire team of specialists providing relief when a cure for the illness is not possible, and is focused on relieving and reducing the symptoms of the disease while maintaining the highest possible quality of life.
 

Types of Hospice Care Services

Your Care Plan

Individuals served by the Hospice Team include patients, family members and other caregivers. A specific plan of care, unique to the needs of every patient, is developed by the End-of-Life Care specialists on your team.

The Hospice Team includes the following professionals: Medical Director, Registered Nurses, Certified Nursing Assistants, Social Workers, Chaplain, and other specialists as needed, along with many trained volunteers who have a commitment to patient assistance.

 

 

 

Specific services included in both Hospice and Palliative Programs:alt
 

  • Requests for information, assessment, and referrals accepted any day, any time.
  • Emotional support provided by the entire Hospice Team, coordinated through daily team discussions.
  • Spiritual support led by the Chaplain and supported by the entire Hospice Team.
  • Bereavement and grief support for family members and other caregivers from admission to 13 months after the death of their loved one.
  • Consultations and ongoing communication with family members, other caregivers and service providers.
  • Coordination of End-of-Life services with physicians, hospitals, nursing homes and assisted living facilities teams.
  • Assistance with financial matters, funeral arrangements and medical benefits management.
  • Trained Volunteers supplement the delivery of services by the Hospice professional team in all settings including respite for caregivers, companionship to the patient, errands and light housekeeping.
  • Advance Care Planning services including assistance in completing the Health Care Power of Attorney document.
  • Grief Support including the “Cup of Hope” Support and Information Group meeting monthly at the Hospice Center.
  • Tools to assist in organizing your caregiving circle utilizing the “Share the Care” resource materials.

HOSPICE ADMISSION CRITERIA

  • Patient has a terminal illness with a possible life expectancy of six months or less if the disease runs its normal course.
  • Curative measures are no longer effective or appropriate.
  • Patient no longer seeking aggressive treatment.
  • A physician order is required.

 

PALLIATIVE CARE ADMISSION CRITERIA

  • Patient has been diagnosed with a terminal or life-limiting illness.
  • Prognosis is greater than six months to live.
  • May still be seeking or receiving aggressive treatment for disease.
  • A physician order is required.
